Cancellation Policy

REQUIRED AGREEMENT PARTICIPANT WAIVER & RELEASE OF LIABILITY I am the parent or legal guardian of the participant identified in this registration and hereby grant permission for my child to participate in BRX Volleyball Club clinics, camps, open gyms, tryouts, lessons, and training activities. ASSUMPTION OF RISK I understand that participation in volleyball and related athletic activities involves inherent risks, including but not limited to falls, collisions, overexertion, sprains, fractures, concussions, serious bodily injury, permanent disability, and, in rare circumstances, death. I voluntarily assume all risks associated with my child’s participation. RELEASE OF LIABILITY To the fullest extent permitted by law, I release, waive, and discharge BRX Volleyball Club, its owners, directors, coaches, employees, volunteers, independent contractors, facility providers, and affiliates from any and all claims, liabilities, demands, actions, damages, or causes of action arising out of or related to participation in BRX Volleyball Club activities, except where prohibited by applicable law. MEDICAL AUTHORIZATION In the event of an injury or medical emergency, I authorize BRX Volleyball Club staff to obtain emergency medical treatment for my child if I cannot be reached immediately. I understand that I am responsible for any medical expenses incurred. HEALTH CONFIRMATION I certify that my child is physically capable of participating in volleyball activities and has no known medical condition that would prevent safe participation, except as disclosed during registration. CODE OF CONDUCT I agree that my child and family members will conduct themselves respectfully toward coaches, staff, officials, athletes, parents, and facility personnel. BRX Volleyball Club reserves the right to remove any participant whose behavior is deemed unsafe, disruptive, or inappropriate. ELECTRONIC SIGNATURE By checking the box below and entering my full legal name, I acknowledge that I have read and understood this Waiver and Release of Liability and agree to its terms. I understand that my electronic signature is legally binding and has the same force and effect as a handwritten signature.
